It is my professional duty to collect information from clients in regard to services provided by Mana Therapies & NYROSallySimmers.  This information may include personal details (eg. Name, DOB, contact numbers and addresses, GP details), health history, treatment provided & progression. I collect this data (for GDPR purposes) under the legal basis of CONSENT (being explicit consent from you, the client) and LEGITIMATE INTEREST for the purposes of providing the health & wellbeing care you have engaged my services to receive. The data (as described above) is in the form of handwritten notes and intake forms and is kept in an individual file and locked in a filing cabinet in my office. None of this information is kept online and all of the information contained within this file is kept private and not shared with anyone else unless with your strict permission (eg. referral to another professional) or in the case of a recognised professional emergency. Appointment information is kept in a paper diary and is kept with me, at my home office or in a clinic environment; the information is not shared with anyone else. Contact details (eg. telephone number, email address, social media that you may have used to contact me) may also be stored on my mobile phone or on my laptop, both of which are password protected & will remain with me, at my home/office or within a clinic or to/from a clinic. Client contact details will only be used for the purposes of the services that I have/am providing you with (eg. appointments, intake forms, ongoing health & wellbeing care) and these details will not be shared with anyone else unless explicit consent has been given by you (eg referral to another practitioner). Past appointment information is kept in my locked filing cabinet and used solely for professional and tax purposes. Client files are kept secure for a period of 8 years (18+) and indefinitely (0-18 yrs) to adhere to current legal and insurance purposes, after which time, files will be destroyed.  You, the client, have the right to view your personal file on request.
